Appointment requests are not guaranteed until confirmed by MAX MD. We may refuse, cancel, or reschedule appointments in our discretion, including for safety, clinical, or administrative reasons. You are responsible for providing accurate information when booking or submitting forms.
PAYMENT, CANCELLATIONS, AND NO-SHOWS
Payment policies, deposits, cancellation windows, and no-show fees (if applicable) may be communicated during booking, intake, or at the time of service. You agree to comply with the policies provided to you in connection with your appointment.
COMMUNICATIONS CONSENT
By submitting forms, requesting an appointment, or contacting us, you consent to receive communications from MAX MD related to your inquiry, scheduling, care coordination, and administrative messages via email, phone, or SMS, consistent with your preferences and applicable law.
SMS TERMS OF SERVICE
MAX MD Medical Aesthetics + Wellness may use SMS text messaging to communicate with clients. The following terms apply to all SMS communications.
A) SMS CONSENT AND INFORMATION SHARING The information obtained as part of the SMS consent process, including phone numbers, will not be shared with third parties or affiliates for marketing purposes under any circumstances. IN NO EVENT WILL MAX MD'S TOTAL LIABILITY EXCEED ONE HUNDRED U.S. DOLLARS (US $100) OR THE AMOUNT YOU PAID TO MAX MD FOR THE SPECIFIC ONLINE TRANSACTION GIVING RISE TO THE CLAIM, WHICHEVER IS GREATER, UNLESS APPLICABLE LAW REQUIRES OTHERWISE.
INDEMNIFICATION
You agree to defend, indemnify, and hold harmless MAX MD and its affiliates, providers, employees, and agents from and against any claims, damages, liabilities, losses, and expenses (including reasonable attorneys' fees) arising from your use of the Site, your violation of these Terms, or your violation of any rights of another.
